Death Cards

Death Cards are created upon dying in a unique way. On the Log screen, it will show the image and play the sound associated with each death. There are 60 death cards (3 are initially hidden).
A death card can be awarded with a corresponding medal based on the number of said death :
A Golden Medal is awarded to the card with the most deaths.
A Silver Medal is awarded to the card with the second most deaths.
A Bronze Medal is awarded to the card with the third most deaths.
Note that if more than one card have the exact same number of deaths, if eligible, they will all be awarded the same medal.

2 | Fall onto a spike trap. | |
3 | Having a push block fall onto you. This death ignores invulnerability and will end your run immediately no matter how many revive potions you are carrying. | |
4 | Be killed by a Hound. | |
5 | Be killed by a fish in a flooded floor. | |
6 | Be killed by a Jellyfish in a flooded floor. | |
7 | Be hit by a worm in The Hatchery.
Worms can only catch you when they first come out of the ground. |

8 | Get more than 4 jump auras at a time. This death ignores invulnerability. | |
9 | Be hit by a lava drip. Lava drips splash out of lava pools when an object is thrown into them, when a block holding lava is broken, or when a lantern is broken. In Chapter 2, lava drips can come out of various other sources. | |
10 | Fall into lava. This death ignores invulnerability and will end your run immediately no matter how many revive potions you are carrying. | |
11 | Be killed by a falling icicle. Icicles can be triggered by breaking a block. | |
12 | Be killed by the spinning seeds in Conservatory. | |
13 | Be killed by a spike that comes off of a spike block. The block with spikes on it has to be broken for the spikes to come off of it. | |
14 | Touch a Beetle in the Hatchery. The larvae themselves are harmless. | |
15 | Take too long in the final stage of the Krystelle fight. | |
16 | Touch any part of the giant worm. | |
17 | Go into the gas that appears randomly in Conservatory. | |
18 | Touch a wall or ceiling covered in death plants in Ascension. | |
19 | Touch a Rat. | |
20 | Climb on the outside of the tower during rain. This can only happen in Soul Crossing and Ruins. | |
21 | Fall out of the tower. This can only happen in Soul Crossing, Ruins, and Zardoland. This death ignores invulnerability and will end your run immediately no matter how many revive potions you are carrying. | |
22 | Touch a spinning fan in Fatal Flight or Soul Crossing. | |
23 | Killed by a Spirit in Soul Crossing. | |
24 | Get crushed by a falling trap in Segmentation Plant. This death ignores invulnerability and will end your run immediately no matter how many revive potions you are carrying. | |
25 | Get shot by a laser trap in Segmentation Plant. | |
26 | Stand on a spike trap in Segmentation Plant. | |
27 | Touch a Beeble in Volt Caves. | |
28 | Touch an ionized block in Volt Caves. | |
29 | Stand near a mine in Zardoland when it blows up. These mines will spray lava drips in a circular radius when they explode. | |
30 | Stand near a grenade thrown by a blue Goon in Zardoland. | |
31 | Be punched by a pink Goon in Zardoland. | |
32 | Lose to Zardo in Z-Ball. This death ignores invulnerability and will end your run immediately no matter how many revive potions you are carrying. | |
33 | Touch a glowstick thrown by a dancer in Club Z. | |
34 | Touch the magic left by a raver in Club Z. | |
35 | Touch a red block in Arcade. | |
36 | Let a worm bleed you out in Temple of Devotion. | |
37 | Be grabbed by a Watcher in Temple of Devotion. | |
38 | Get crushed by a devote in Temple of Devotion. | |
39 | Touch the Mother Hound in Coliseum. | |
40 | Get hit by Brobash's Scimitar in Coliseum. Brobash's corrode potions can't kill you. | |
41 | Stand under a guillotine in Zenith. | |
42 | Stand by a fan blade in Zenith. | |
43 | Stand at a fan in Zenith. | |
44 | Get killed by a laser beam from a stone eye in Zenith, or by the final bosses in Colossus. | |
45 | Get shot by one of Zardo's magic blasts. | |
46 | Get shot by a large magic blast from Zardo. | |
47 | Get shot by a magic bolt during the fight with Zardo. | |
48 | Get caught under one of Zardo's hands. | |
49 | Get hit by a magic ball from Zardo. | |
50 | Get hit by a laser in the final phase of Zardo's fight. | |
51 | Stand within the blast radius of a powder keg. | |
52 | Destroy a block in Club Z, causing the bouncer to come after you. This death will end your run immediately no matter how many revive potions you are carrying. | |
53 | Get killed by Krystelle in Chapter 2 (it doesn't matter how). | |
54 | Get killed by a Corrupted Watcher in Temple of Devotion. | |
55 | Get stabbed by the skewers in Temple of Devotion. |

Trivia
- The only enemies that cannot directly kill you are Zardo in Chapter 1, Bats, Wasps and orange Goons.
- On easy mode, being crushed does not instantly kill you; the blocks will break upon collision and you are allowed to continue the run.
